BRAZORIA COUNTY DISEASE SURVEILLANCE
REPORTABLE/NOTIFIABLE CONDITIONS
The Texas Administrative Code, Title 25 requires hospitals, health care providers, laboratories, and schools to report confirmed or suspected cases of the conditions listed below to public health authorities. Persons employed in child care facilities, nursing homes, home health agencies, and food-handling establishments are also encouraged to report. Furthermore, any unusual disease cluster or condition of public health concern should be reported. Reports received by the Brazoria County Health Department will be investigated as appropriate. Information on confirmed cases will be reported to the Texas Department of Health as required.
WHAT TO REPORT
Please complete the front side of the Brazoria County Health Department DISEASE REPORT FORM (downloadable Word format) as completely as possible. The minimum information should include name, birth date, sex, race/ethnicity, address, telephone number, disease/condition, onset date, health care provider, and method of diagnosis.
